ABSTRACT

Objective@#To explore the effect and safety of fractional 1565 nm laser on white striae gravidarum (SG) in abdomen and observe the influencing factors of treatment effect.@*Methods@#Thirty-six patients, aged 24-39 years old, were randomly divided into two groups(group A, n=16, group B, n=20): patients in group A underwent two passes of laser treatment: striae were locally scanned at moderate level energy(35 mJ/μb) with high density(300 dot/cm2), and high level energy (45 mJ/μb)with low density(150 dot/cm2)was adopted for the whole area.Group B received one pass of treatment at moderate energy with high density 35 mJ/μb, 300 dot/cm2). All patients received three laser treatments at a 6 weeks interval. Improvement of SG appearance was assessed by blinded reviewers, 3 months after the last treatment, using a percentage category scale. Meanwhile, patients satisfaction was evaluated. The impact of skin aging on laser treatment was evaluated. Patient satisfaction scores and side effects were evaluated.@*Results@#All patients completed the overall trial. The average improvement of scores was 1.69±0.95, which contributed to a fairly good response rate of 88.89%. The patients satisfactory and very satisfactory rate was 58.3%(21/36). There was no significant difference in improvement score between group A(1.88±0.89) and group B(1.55±1.00) (P< 0.01). The operation duration of group B was shorter than group A, (29.6±4.9)min vs.(53.7±5.5)min, P<0.05. There was a moderate negative correlation between aging and laser treatment effect (r=-0.553). The abnormal color of skin(erythema and post-inflammatory hyperpigmentation) in group A lasted longer than group B, (9.17±4.03)d vs. (6.73±2.29)d, P<0.05.The pain score during treatment was tolerable, with the score of 2.61±0.76.@*Conclusions@#The treatment with fractional 1565 nm laser can improve the appearance of SG. It is more timesaving when the entire implicated area treated by one pass at moderate level energy with high density. Skin aging will discount the effect.

ABSTRACT

Objective To evaluate the elimination of melasma with nonablative Q-switched Nd : YAG1 064 nm laser. Methods 23 cases of melasma were treated with Q-switched Nd :YAG 1 064 nm laser using the following parameters: spot size, 6 mm; pulse duration, 5-7 ns; frequency, 10 Hz, and flu-ence, 1.8-2. 0 J/cm2. The procedure was repeated weekly. Results The treatment was well tolerated without the use of topical anesthetic. There was slight congestion, no blister and crust after treatment. Routine clean and make-up can be done after treatment. The melasmas were obviously lighten or fully eliminated after 8-10 treatments and 52. 17 % of patients showed excellent results. Only 5 cases experienced an episode of purpura on eyelid after treatment and disappeared spontaneously without post-inflammatory hyperpigmentation and scarring. Followed up over 6 months, there was recurrence in 7 patients who were effective by additional treatment. Conclusions Nonablative Q-switched Nd :YAG 1 064 nm laser is a more ideal method to eliminate melasma because of the credible effect, safe, simple operation, and no downtime, in despite of recurrence.

ABSTRACT

<p><b>OBJECTIVE</b>Gene therapy has been becoming one of the most attractive medical areas. But the using of gene therapy in plastic surgery is relatively scarce. Our purpose was to investigate the effect of naked plasmid encoding Vascular Endothelial Growth Factor on the viability of the random skin flap by directly injected subcutaneously.</p><p><b>METHODS</b>30 female Sprague-Dawley rats randomly divided into three groups. A random dorsal skin flap of 3 cm x 9 cm was elevated in each of the rats. And 1 ml double-distilled water solution was injected subcutaneously, which was only water in group 1 during the operation, 200 micrograms VEGF cDNA plasmid in group 2 during the operation, 200 micrograms pcDNA3.1/zeo(+)--VEGF in group 3, 24 hours before the operation, respectively. 7 days after the operation, all the animals were sacrificed by overdose anesthetic. The survival tissue was measured with planimetry. Two samples were harvested from each group for pathological check and immunohistochemical test.</p><p><b>RESULTS</b>Immunohistochemical staining demonstrated that there was human VEGF deposited around the capillary in the flaps treated with VEGF gene. The flaps treated with VEGF gene had a larger percentage of survival skin (group 1 = 47% +/- 5.4%, group 2 = 65.4% +/- 6.3%, group 3 = 72.3% +/- 8.5%, P < 0.05).</p><p><b>CONCLUSION</b>VEGF gene directly injected into subcutaneous can express VEGF. It makes the gene therapy simple and practical and will be promising future in the tissue transplantation.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the safety of Hydrophilic polyacrylamide gel (HPAG) through an animal experiment.</p><p><b>METHODS</b>After HPAG was injected underneath the skin of SD rats, tissue specimens were taken for general and histological examinations. The cytotoxicity was evaluated by agar coverage and MTT method.</p><p><b>RESULTS</b>It was determined that the cytotoxicity was over level-two. The toxicity to kidney was obvious. The local histological reaction was slight and a thin fibrous membrane was formed around HPAG, which became stiff gradually. The shape and location of the injected HPAG was not stable. The HPAG could not be drawn out completely.</p><p><b>CONCLUSION</b>HPAG has obvious cytotoxicity and is not a suitable material as soft tissue implant for the bad shape and texture.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To summarize clinical experiences in fat particle injection auto-transplantation during the past ten years.</p><p><b>METHODS</b>Retrospective analysis of 334 cases of fat particle injection auto-transplantation was done, and we suggested the correct method of liposuction and fat injection.</p><p><b>RESULTS</b>In this series, one patient (0.29%) had a complication, thirty one patients (9.38%) had lower survival of autogenous fat-transplantation.</p><p><b>CONCLUSION</b>In order to improve the results of fat-grafting, we must adopt the correct method of liposuction and follow the right rules.</p>

ABSTRACT

Objective To evaluate the hair-removal effect of intense pulsed light (IPL) on hirsutism of spina bifida occulta. Methods 8 patients with hirsutism of spina bifida occulta were treated 3 to 5 times, at 2 month intervals, with ELITE plus IPL. Its wavelength was 610~1000 nm, and spot size was 50 mm ? 10 mm. The macro pulse was composed of a series of micro pulses with equal intervals. The micro pulse duration was 5 ms, the number of micro pulse was 4-15, the interval of micro pulse (delay) was 2~20 ms, and the fluence was 22.3~38.3J/cm2. Results All of the patients were well tolerant under no anesthesia. The hairs were mostly or fully removed after 3 to 5 courses of treatment. Routine washing could be done and bandaging was not applied after treatment. There was no blister, infection, hyperpigmentation and scarring. Little vellus hair regrowth was observed after a 6~12 month follow-up. Conclusions IPL is an ideal method to depilate hirsutism of spina bifida occulta because of its credible effect, simple operation, rapid treatment and no serious complication.
